A new study from the Tulane National Biomedical Research Center suggests that a compound found in some vegetables may help support gut repair after HIV-related damage. The study, published in JCI Insight, found early evidence that diet-derived compounds called indoles may help restore immune activity tied to gut lining health, even after long-term antiretroviral treatment.
Why Gut Damage Lingers
The gut plays a major role in immune health, which is why HIV can have effects there early and often. A large share of the body’s immune cells live in the gut, where they help keep the intestinal lining strong and protect against harmful microbes. When HIV attacks those immune cells, it can damage the gut’s protective barrier and weaken its ability to do that job well.
Over time, that damage can make the gut more permeable, allowing microbial products and other substances to leak into the bloodstream. This can keep the immune system activated even when the virus itself is controlled by treatment, contributing to ongoing inflammation in the body. In other words, the virus may be under control, but the gut may still be healing. This is often referred to as “leaky gut” syndrome.
This is one reason researchers are interested in how the gut might be repaired after HIV-related damage. If the lining can be strengthened and immune activity restored, it may help reduce inflammation and support better long-term health outcomes.
What the Researchers Studied
The Tulane team, led by Namita Rout, associate professor of microbiology and immunology at the Tulane National Biomedical Research Center, studied gut changes in nonhuman primates with SIV, a virus closely related to HIV that researchers use as a model for infection. They found that while long-term antiretroviral therapy successfully controlled the virus, it did not fully restore key immune functions needed to protect and repair the gut lining.
The immune cells affected included gamma delta T cells and innate lymphoid cells, which normally produce signaling molecules that help protect the intestinal lining and support tissue repair. In the treated animals, those protective responses were reduced and linked to changes in proteins that help direct immune activity needed to maintain the gut barrier.
The Vegetable Connection
Researchers then tested whether that pathway could be influenced through diet. They gave a small group of animals a broccoli-based supplement designed to increase exposure to indoles, which are naturally found in vegetables such as broccoli and cabbage.
The idea was to see whether boosting these diet-derived compounds could help support the immune responses that protect the gut lining. After one month, the animals showed signs of improved gut barrier integrity and shifts in immune cells linked to mucosal repair. This suggests that the gut may still respond to nutritional support, even after long-term HIV treatment.
While these findings are promising, they do not show that broccoli supplements or related compounds are a treatment for people with HIV. The study was small and done in animals, so more research is needed before any conclusions can be made about the impact in humans.
Why the Study Matters
Even so, the study helps scientists better understand why gut damage and inflammation can persist after HIV is controlled. It also points to a possible immune pathway that future nutritional strategies may be able to target to support long-term health outcomes.
